It doesn't seem that I can get the parental relationship of a custom activity record created from contact to display the associated account (company name). I believe this is due to custom activity entities already create relationship to account so I am unable to find a workaround. It would be nice to automatically show the associated account name and run report by account for this custom activity when creating the activity from (related to) a contact record.

Here is a demo.
I imitated your form layout. Note that the field 'Company Name (Regarding Account)' should be a lookup field for Account instead of a field of type Customer.
First, create a new N:1 relationship for the custom activity entity and Contact entity. Then add fields mapping for this new relationship, map 'Company Name' and 'Company Name (Regarding Account)'.

When we create a custom activity form a contact, the field 'Company Name (Regarding Account)' will be automatically populated with the value of the contact's field 'Company Name'.
The reason for creating a new relationship with Contact entity is that the original relationship with Contact entity (Regarding) cannot add field mappings, so we have to create a new relationship to add field mappings.

Could you provide a screenshot of your custom activity form?
Are there separate lookup fields for contact or account?
Because without them, when you create an activity from a contact, the Regarding field of the event is the contact, so there is no lookup field for the related account.
In general, we can achieve your needs through field mapping or workflow. Using them does not require you to manually select an account, the account field will be automatically populated.
If I know your custom activity form, I will write a demo for you.
If there are no other account or contact fields other than the Regarding field, then you could only use the quick view view to display the account.
Made sense. I inserted onto the Custom activity entity main form a quick view form from accounts that shows company name. It doesn't seem to want to display data. Lookup field = Account, Related entity = Account, appropriate quick view form with Company Name field. Any thoughts.
To summarize situation, I am merely creating a custom activity entity nothing special and then creating an activity that relates to the contact, wanting the account relating to the contact to show. We do not want to have to add an account lookup field and manually associate account if possible but rather have it show in lookup field or view,etc. automatically based upon contact's association with account (company name).

Do you want to see related account of the contact set in the custom activity form ?? one possible option should be to use Quick view for the account entity and place it on your custom activity entity where you can show related account based on the contact lookup
It's the same functionality as like when a new opportunity is created related to a contact the parent account (company name) also is automatically included. So when a new activity (via this custom activity entity) is created related to a contact the associated parent account (company name).
A custom activity entity automatically create a relationship with account so I not sure if we can get around that to show the associated account (I believe parental relationship) of the contact on the custom activity form. I can create a lookup field for account and relate it manually but I want to show it on the form based upon the contact
Can you add screenshot or some more explanation of what you are trying to achieve?
